Health
Insurance
The
medical center offers the following three health insurance
plans for 2003. For more information on these plans, check out
their web sites by clicking on these links:
Family
Status Changes Once
you choose your benefits for the coming year, you may not change your
elections unless you have a qualifying family status change.
Family status changes include:

marriage,
divorce, or legal separation
-
birth
or adoption of a child
-
death
of a spouse or dependent child
-
dependent's
loss of eligibility
-
change
in your spouse's employment status (hired, terminated, change from
full-time to part-time or vice versa)
-
your
spouse's loss of eligibility under another health care plan
-
leave
of absence
Family
status changes must be made within 30 days of the qualifying event.
